The West Virginia Parole Board requests receiving states include an affidavit that is signed by the arresting officer when returning offenders. Please attach the affidavit to the arrest report in ICOTS when returning offenders who demonstrated behavior that resulted in his or her arrest in the receiving state.

In part, per the West Virginia Parole Board Procedural Rules:

d. 1.     The document is prepared by a person with direct knowledge of relevant facts, who is unable to appear to testify, provided that the document is accompanied by an affidavit signed by the author attesting to the document’s authenticity and accuracy or there are indications that the document is reliable and trustworthy.
